How to receive care in your home

We know that there is nothing better than being able to live life the best you can in the safety and comfort of your own home. For most people, home is a major part of life where a deep sense of freedom and independence is realized. We understand that this is important for you at any stage of life. We also understand that you may have personal reasons why continuing to live at home is exactly what you prefer.

Step 1: Request a Needs Assessment (NASC)

If you have a disability or ongoing health problems, ask for an assessment to see if you can get help with receiving care in your home.

Step 2: Assessment process
NASC will conduct the assessment. An assessor will visit you to find out what you need help with.

Step 3: Plan your Home care
The Needs Assessment Service Coordination (NASC) team develops a care plan or home support plan based on the information you’ve given them.
Step 4: Sign up with your preferred care provider

Scott Holland will visit you to explain the service agreement, the care packages you prefer and when to commence the service.

If you have a disability or ongoing health problems, ask for an assessment to see if you can get help with receiving care in your home.
 

The government provides help if you need help with your personal care because you:

  • have ongoing or age-related health problems
  • live with a disability
  • have an injury or disability because of an accident
  • are recovering after hospital treatment and need help for a short while.
Arrange a needs assessment through your doctor, District Health Board (DHB) or Needs Assessment Service Coordination (NASC) team. Step 2: Assessment process
NASC will conduct the assessment

An assessor will visit you to find out what you need help with. You can invite family, whānau or someone else to support you during the meeting. If your situation is straightforward, the assessor might contact you by phone first.

The assessor will want to know about your health, any support you have from family and friends, how you cope with day-to-day tasks, and how you’re feeling about the future.

The Needs Assessment Service Coordination (NASC) team develops a care plan or home support plan based on the information you’ve given them. They will let you choose from a list of accredited home care providers, or you may request your preferred one.
